Gold Coast Health Plan (GCHP) is pleased to release its Pathways to Wellness (PTW) Request for Applications (RFA) through its Community Health Investments grantmaking program. Este programa se estableció en 2017 con el fin de otorgar subsidios a organizaciones externas que abordan los factores sociales determinantes que afectan la salud y el bienestar de los miembros de GCHP.

The goal of the Pathways to Wellness (PTW) Community Grant Program is to invest in outreach efforts aimed at helping members navigate their health care journeys by addressing the social, cultural, structural, and environmental barriers that prevent them from accessing the care and services they need.
