Members who complete certain health screenings and preventive care exams can earn gift cards.
How to get your reward:
- Complete your qualifying service.
- Fill out the reward form, and ask your provider to sign it as directed.
- Send the form by mail, fax, or email to GCHP. Be sure to include the address where your gift card should be sent.
Some clinics may provide the gift card at the time of your visit. Please check with your clinic.
*Services must be completed between Jan. 1, 2026, and Dec. 31, 2026. One gift card per member, per program.
