Gold Coast Health Plan (GCHP) members who are 21 or older and have both Medicare and full-scope Medi-Cal can enroll in Gold Coast Health Plan Total Care Advantage (HMO D-SNP). This plan, known as a Medi-Medi plan, brings Medicare and Medi-Cal benefits together into one coordinated program.
