Members 40-74 years of age who complete a mammogram can earn a $50 gift card from either Target or Walmart. Download the form for more information.

How to earn your gift card:
- Be enrolled in Total Care Advantage when you complete your visit, screening, or test.
- Complete your health visit, screening, or test between Jan. 1, 2026 and Dec. 31, 2026.
- Mark your completed activities and return the filled-out form to Total Care Advantage.
- Send the form to Total Care Advantage by Jan. 31, 2027.
