To simplify and streamline the delivery of services to children and youth under 21 years of age, and in alignment with the goals of the California Advancing and Innovating Medi-Cal (CalAIM) initiative, the state Department of Health Care Services (DHCS) phased out the Child Health and Disability Prevention (CHDP) program. These services were transitioned to other delivery systems, effective July 1, 2024.
Prior to July 1, 2024, the CHDP program included:
- Responsibility for provider trainings on well-child preventive care screenings that included anthropometric measurements, dental screenings / fluoride varnish application, and audiometric and vision screenings.
- Responsibility for facility and medical record reviews of CHDP program provider sites.
Effective July 1, 2024:
- Managed care plans (MCPs) shall ensure that ongoing provider trainings on well-child preventive care screenings outlined above are conducted.
- MCPs shall conduct the facility site and medical record reviews (FSR/MRRs) of former CHDP program providers who are also primary care providers seeing Medi-Cal members during their periodic managed care FSR/MRRs.

Staff Competency Checklist
Please complete the checklist to assess staff competency for:
- Anthropometric measurement
- Fluoride varnish application
- Audiometric screening
- Vision screening
Medical Assistant Attestation
An attestation form providers may use as evidence that individuals have successfully completed the well-child preventive care screening trainings. DHCS requires these trainings be completed every four years. The attestation also covers other required training topics (such as venipuncture and administering injections).
