Gold Coast Health Plan invests in Ventura County health care organizations within its network through RISE grants, which support Medi-Cal members in understanding, navigating, and complying with evolving Medi-Cal eligibility, work, and community engagement requirements.

El Programa de Subvenciones Resiliencia, Innovación, Sostenibilidad y Equidad (RISE) de GCHP es un programa de tres años destinado a ampliar el acceso a la atención para satisfacer mejor las necesidades de salud física y del comportamiento de los miembros de MEDI-CAL en el condado de Ventura.
Objetivos y Propósito
El objetivo del Programa de Subvenciones RISE es mejorar de manera medible la calidad y el acceso a la atención médica y de salud conductual para la población MEDI-CAL de GCHP en el condado de Ventura. In 2026, the program seeks to proactively prepare providers and community partners to support Medi-Cal members in navigating upcoming eligibility and engagement requirements anticipated to begin in 2027 - with the goals of minimizing coverage disruptions, improving member experience, and promoting continuity of care.
In Year 2, GCHP will offer one-year grant awards of no more than $400,000, focused on the strategic priority described below. We are seeking a range of application budgets that represent different scopes of work.
Year 2 Strategic Priority
Support Medi-Cal members in understanding, navigating, and complying with evolving Medicaid eligibility, work, and community engagement requirements.
This may include provider- and community-led initiatives that help members:
- Maintain Medi-Cal coverage
- Avoid administrative disenrollment
- Successfully complete renewal requirements
- Connect to qualifying work, training, education, or community engagement activities through culturally and linguistically appropriate navigation, community or volunteer programs and opportunities, and education support

The RISE Grant Program will open for applications on Wednesday, Feb. 18, 2026, and will close for submissions on Wednesday, April 15, 2026. Las subvenciones se anunciarán el lunes 1 de junio de 2026.
The grant award period will run from July 1, 2026, to June 30, 2027.
